Welcome to all new students and parents. The following medical information will assist you in preparing for your child's entrance into school.

All children need a current immunization record. Starting this school year (fall 2005) all 6th grade students need to have a varicella (chickenpox) vaccine or provide proof that they have had the disease. All kindergarten students will need to have a second measles vaccine prior to the start of the school year.

A health inventory form needs to be completed and returned to school if you are new to Bishop Dunn.

A pink emergency card will be mailed to you over the summer. Please complete the card and return it to school, preferably on the first day.

Should your child need to take medication during the school day, we need a written order from the health care provider, and the medication needs to be sent into school in the orginial pharmacy container. You may download a medication permission form by clicking on the tab on the left hand side of the website page (directly under "Health Office").

Please keep the health office informed of any changes in your child's medical status.

All students who are new to Bishop Dunn are required to have a physical examination. This is also a requirement for students in grades 2, 4,and 7.If your child will be participating in our sports or cheerleading program they are required to have a physical exam too. At the present time the Newburgh School District is not able to provide physical examinations. Please contact your Health Care Provider.
